The Department of Health Services in collaboration with local partners is inviting members and providers of Community Health Partnership to attend an information session regarding the transition to a new managed care organization. The session will be held Tuesday, Oct. 30, from 2-4 p.m. and 6-8 p.m. at the St. Croix County Government Center, 1101 Carmichael Road.

During the sessions members and providers will hear about the program choices available and will have an opportunity to hear from and meet with the new agency that will be providing Family Care in the area. There will also be representatives from Aging and Disability Resource Centers and an Ombudsman that can also answer questions.

"In an effort to ensure a smooth transition, we are hosting informational sessions that members and their families and guardians, as well as health care providers can attend," said Deputy Secretary Kitty Rhoades. "However, members do not need to attend a meeting in order to enroll into a new program."
